Two men in Shanghai were confirmed to have died from a new strain of bird flu, official media reported on Monday, prompting fears that Chinese health authorities were trying to play down a public health threat.

The two men died from the H7N9 avian influenza strain in early March, while a woman in Nanjing remains in critical condition, but Beijing waited nearly three weeks before making an announcement.

One of the men was 87 years old, and died on March 4 after being taken ill on Feb. 19, while the second died on March 10 after becoming ill on Feb. 27, the official China Daily newspaper quoted health officials as saying.

The Geneva-based World Health Organization (WHO) said it was monitoring the situation.

"There is apparently no evidence of human-to-human transmission, and transmission of the virus appears to be inefficient, therefore the risk to public health would appear to be low," regional agency spokesman Timothy O'Leary told reporters.

Experts are still unsure of how the three people contracted the virus, although officials don't believe they caught it from each other.

An employee who answered the phone at the Shanghai No. 5 People's Hospital declined to comment on the cases, as did an official in the press office of the health and family planning ministry.

"You can keep looking at our official website," she said. "Right now we don't have any relevant information to announce."

'No common source of infection'

Hong Kong's secretary for food and health, Ko Wing Man, said the Special Administrative Region (SAR) government had already been officially informed of the new bird flu cases.

"So far, there is no common source of infection for these three cases," Ko said. "It could be pigs, but it could also be poultry."

"It seems that there wasn't a single source of infection, which is worrying and a cause for concern," he said.


Meanwhile, Hong Kong University infectious disease expert P.L. Ho said it was unusual for a virus without the H5 prefix to infect humans.

"These are special circumstances, in which the virus has not only infected humans, but has resulted in fatalities," Ho said.

"From a virological point of view, there is a possibility that the virus will mutate further," he said.


The news prompted concerns among residents of Shanghai, who are still reeling after tens of thousands of dead pigs were pulled out of a river that supplies their drinking water.

"Now, everyone is joking around that when you go out to eat, you can't eat poultry now, either," said a resident surnamed Yao. "We also had the dead pigs in the Huangpu river, and now they're saying that some of the diseased pigs were used to make steamed rice parcels."

The Hong Kong Oriental Daily News ran an editorial on Monday calling on Chinese health officials to provide people with timely and accurate information on the disease.

"The authorities waited more than 20 days before they announced this," the paper said. "It makes you wonder whether it was because they were holding the parliamentary sessions in Beijing at that time, and they wanted to cover up the truth at that time."

Announcement delayed?

Guangzhou-based rights lawyer Tang Jingling said the authorities had probably delayed the announcement out of concerns over social stability.

"The main reason is the 'stability maintenance' thinking of the authorities," Tang said. "They figure that if they announce it, the job of maintaining social stability will get a lot harder."


"Staying in power matters more to the [ruling] Chinese Communist Party than the safety of its people or their property," he said. "A lot of the information they do put out has been toned down; so they might for example cut the number of people infected and the true geographical extent of the infections."

"I don't think they would be open about these things."

Chinese public health procedures have come under intense scrutiny and suspicion since an official attempt to cover up the extent of the SARS epidemic of 2003 was exposed by a military doctor in Beijing.

“There is no connection between all the reported cases of H7N9 bird flu,” Su said, “which might suggest that the epidemic has spread to many different areas.
“Because the symptoms connected to the first stage of H7N9 are not obvious, it might be hard to prevent the epidemic from spreading to Taiwan,” said Su.
Centers for Disease Control (CDC) Deputy Director-General Chou Chih-hau (周志浩) said health inspection of travelers entering and leaving the country at airports and ports has been enhanced.

China's National Health and Family Planning Commission on Sunday notified the World Health Organization (WHO) of three cases of human infection with H7N9 bird flu in China, the WHO stated Monday.

The health organization said the Chinese government was engaged in an active investigation and had instituted enhanced surveillance, laboratory strengthening and training of healthcare professionals for detection, reporting and treatment.

"China actually for a long time has been reporting promptly and openly. I think SARS was a turning point globally for that sort of thing. Not just in China," he said.
He was referring to the 2003 epidemic of Severe Acute Respiratory Syndrome, which caused a scandal when official numbers of cases were later dramatically revised upwards.
SARS emerged in China and killed about a tenth of the 8,000 people it infected worldwide.
People buying poultry at a Shanghai market on Monday said they were worried the incidents had been made public only weeks after the deaths and said they hoped the government would be quick to report any new cases.
"In the future, no matter what it is, the government should make it public quickly and let the people know early. That way they can prevent it themselves", said shopper Zhang Zhili, 60.
Many users of Chinese microblogs, known as "weibo", said they suspected the latest outbreak of bird flu was related to more than 16,000 pig carcasses recently found dumped in rivers around Shanghai.
O'Leary said that while the dead pigs were part of the overall investigation, there was no evidence of any connection.
It is not known how the three victims were infected. The virus does not seem highly contagious because no health abnormalities were detected among 88 of the victims' close contacts, the health commission said. The 87-year-old's two sons have gone to hospital with similar symptoms.

The above comments describe family members of the first confirmed case (87M, A/Shanghai/1/2012) who developed pneumonia.  The confirmed case, as well as one of his sons, died.  The cluster raises concerns that H7N9 is transmitting human to human (H2H) which is not being identified with current testing procedures.  It is likely that all three were H7N9 infected.  The sequence for A/Shanghai/1/2013 was more diverse than A/Shanghai/2/2013 and Anhui/1/2013, but all three had PB2 E627K as well as the 15 BP deletion in N9, signaling human adaptation.

Disease onset dates for the sons would be useful.  H7 has also been identified in horses, supporting mammalian transmission, and other human cases have been associated with symptomatic contacts.  Serological studies had also signaled human transmission, although prior clusters have been mild.  The only prior fatal H7 case (H7N7 in Netherlands in in 2003) had PB2 E627K, which was present in all three 2013 confirmed H7N9 cases, which were severe or fatal.

 (AP) — Health officials say they still don't understand how a lesser-known bird flu virus was able to kill two men and seriously sicken a woman in China, but that it's unlikely that it can spread easily among humans.
Two men in Shanghai became the first known human fatalities from the H7N9 bird flu virus after contracting it in February. A woman in the eastern city of Chuzhou remains in serious condition, China's National Health and Family Planning Commission said.
It was unclear how the three patients became infected, the health agency said. It sought to calm fears about the virus but provided few details about each case. Authorities have not described the patients' occupations or said whether they had come into contact with birds or other animals.
The health authority noted, however, that two sons of one of the Shanghai men also suffered from acute pneumonia, and the source of their infection is still unknown. The Chinese health agency said other people who were in close contact with the victims have not become sick, indicating that the virus is not easily transmitted between humans.
"We don't know yet the causes of illness in the two sons, but naturally, if three people in one family acquire severe pneumonia in a short period of time, it raises a lot of concern," the World Health Organization's China representative, Michael O'Leary, said at a briefing in Beijing late Monday.
Other strains of the H7 family of bird flu viruses have caused mostly mild human infections in the past, said University of Hong Kong microbiologist Malik Peiris, with cases reported in the Netherlands, Canada, the U.S. and Britain — mostly following outbreaks in poultry.
Experts say the deaths in China might indicate that the H7N9 strain has morphed to become more lethal to humans, although it's not possible to make any conclusions yet about its mortality rate because many mild cases may go undetected. A thorough tracing of the virus is critical.
"I would guess that given the severity of the human disease it is likely that these particular viruses have undergone the change to become highly pathogenic but obviously that remains to be ascertained," Peiris said. "The crucial question is the source of this virus, where is it."
Scientists have long feared that another bird flu strain, H5N1, might mutate to spread more easily. But while it has decimated poultry stocks mostly in Southeast Asia, it has only occasionally sickened people — mostly after close contact with infected poultry. It can be deadly when it does infect humans, though, killing about 60 percent of the time.
"At the moment, there has been not much evidence of human-to-human transmission (of H7N9) so to that extent it is similar to the H5N1 situation, but it is early days and so there's a lot more to be understood," Peiris said.
More than 16,000 pig carcasses were fished out of the river system that supplies some of Shanghai's water supply in March, apparently dumped by farmers after they were sickened, and some observers have wondered whether there might be a link between the sickened pigs and the bird flu deaths.
However, O'Leary said the H7N9 infections pointed to an avian flu, not a pig disease, so he said he thought the timing of the pig deaths was coincidental. But he said that epidemiologists were looking at any possible avenue of infection.
Scientists classify flus based on the proteins on the surface of the virus: There are 17 varieties of hemagluttinin, the H in a flu's name, and 10 varieties of neuraminidase, the N component. Any combination of those Hs and Ns could crop up and potentially mutate into a form that's spread easily from person to person, making it dangerous enough to produce a pandemic.
Health authorities are monitoring 88 people who came into contact with the H7N9 patients and have not found any additional infections so far, China's health agency said. Experts say that indicates that the chance of human-to-human transmission is low.
"It is very unlikely, because the virus has to break the species barrier and this is usually quite a difficult event. There has to be a lot of significant mutation," said David Hui, an infectious disease expert at the Chinese University of Hong Kong.
Many epidemiologists regard densely populated parts of China and Southeast Asia, where farmers often live in close quarters with pigs and poultry, as regions where conditions are ideal for nurturing infectious diseases that jump from animals to humans. An earlier deadly outbreak of SARS, or severe acute respiratory syndrome, was linked to wild animals that infected animals, which in turn infected people in that region.
One of the male victims of H7N9, who was 87, became ill on Feb. 19 and died on Feb 27. The other man, 27, became ill on Feb. 27 and died on March 4, the Chinese health commission said. A 35-year-old woman in the Anhui city of Chuzhou became ill on March 9.
Timothy O'Leary, the World Health Organization's regional spokesman in Manila, said the risk to public health appears low because there is no evidence of human-to-human transmission, but that WHO is closely monitoring the situation.
"Obviously we're very concerned about the evidence that humans have not only become infected with H7N9, but also have died as a result of it," O'Leary said.
